HIPAA Policy & Other Notices
HIPAA stands for the Health Insurance Portability And Accountability Act and protects your rights as a patient including who you designate to receive information about your health care through the Protected Health Information – or PHI – regulation.
Click below for a copy of the policy as well as a form you can use to update your PHI disclosure information. The form allows the Fund Office to share information or assist an authorized person – such as a spouse or close relative -- with the member's health coverage, for example, during an emergency.
YOU DO NOT HAVE TO RETURN THE FORM UNLESS YOU WANT TO CHANGE OR UPDATE YOUR PHI DESIGNEES.
