HIPAA NOTICE OF PRIVACY PRACTICES FORMS ARE IN THE MAIL

Local 1262 members who have health insurance and other related benefits through our Funds should watch their mail for a Notice of Privacy Practices and forms. This Notice is part of Federal HIPAA regulations.
HIPAA stands for the Health Insurance Portability And Accountability Act and protects your rights as a patient including who you designate to receive information about your health care through the Protected Health Information – or PHI – regulation.
The mailing from our Funds contains a copy of the policy as well as a form you can use to update your PHI disclosure information. The form allows the Fund Office to share information or assist an authorized person – such as a spouse or close relative -- with the member's health coverage, for example, during an emergency.

YOU DO NOT HAVE TO RETURN THE FORM UNLESS YOU WANT TO CHANGE OR UPDATE YOUR PHI DESIGNEES.
Simply file the document with your other health care insurance information.
